I had a look on my system, and it appears that there's no individual for .exe file for PR, it in fact appears to run directly off the BF2.exe with a command string attached.
So. You'll need to create a shortcut on the desktop. In the 'target' box, paste the following (including quotation marks):
"C:\Program Files\EA Games\Battlefield 2\BF2.exe" +modPath mods/pr +menu 1 +fullscreen 1
Then, check that the 'start in' line contains the following (including quotation marks):
"C:\Program Files\EA Games\Battlefield 2"
That should allow you to play PR by double clicking the icon.
If you want the tooltip to display like the initial install of PR, copy the following into the 'Comment' box:
Click here to play Project Reality - NavyMedic is my hero
And if you wanna be anal you can change the icon back to the official PR icon by clicking 'Change Icon...' and then pasting the following into the box:
%ProgramFiles%\EA GAMES\Battlefield 2\mods\pr\readme\ico ns\pr.ico
Once you've done all that, you can thank me for 15 minutes of my time
